Find Injury Care (“Find Injury Care,” “we,” “us,” or “our”) is a health technology and provider-discovery platform designed to help users discover and independently connect with healthcare providers based on information such as specialty, location, services, and provider-reported insurance information.

Find Injury Care is not a hospital, medical clinic, physician practice, healthcare provider, medical practice, insurer, or emergency medical service.

Find Injury Care does not itself diagnose, treat, prescribe for, or provide medical care to patients.

Healthcare services are provided solely by independently practicing healthcare professionals and healthcare organizations.

Find Injury Care allows users to search and review information about participating healthcare providers.

Users are responsible for independently selecting the healthcare provider they wish to contact or request an appointment with.

Unless expressly stated otherwise, Find Injury Care does not select a healthcare provider on behalf of a user, diagnose a user’s condition, or determine which provider or treatment is medically appropriate for the user.

Provider search results may be influenced by search criteria, location, availability, profile information, or paid advertising or sponsored-placement arrangements. Sponsored or paid placements will be identified where applicable.

Information appearing on the Platform—including articles, educational material, provider information, search results, reviews, advertisements, and other content—is provided for general informational purposes only.

Nothing on the Platform constitutes medical advice, diagnosis, treatment, or a recommendation for a particular medical treatment.

You should consult a qualified healthcare professional regarding your individual medical condition or healthcare needs.

Do not delay or avoid seeking professional medical care because of information obtained through the Platform.

Healthcare providers listed on the Platform are independent healthcare professionals or healthcare organizations.

A provider’s presence on the Platform does not constitute an endorsement, certification, guarantee, or recommendation by Find Injury Care unless expressly stated and supported by a stated verification process.

Where Find Injury Care displays a verification designation, the applicable designation will identify what information was verified, such as license or profile information. Verification does not constitute a guarantee of the provider’s quality, medical judgment, treatment, outcome, or suitability for a particular patient.

Insurance information displayed on the Platform may be provided by healthcare providers and may change over time.

References to Workers’ Compensation, No-Fault, PIP, health insurance, or other insurance participation do not guarantee that a particular service will be covered or reimbursed.

Insurance coverage, authorization, eligibility, provider participation, deductibles, limitations, and reimbursement may vary.

Users should confirm coverage and financial responsibility directly with the provider and applicable insurer before receiving care.

The Platform may allow users to submit appointment requests or contact requests to a provider selected by the user.

The provider is responsible for confirming appointments and communicating with the patient regarding availability, treatment, payment, insurance, and other clinical matters.

Where available, Find Injury Care may provide technology that facilitates a connection between a patient and an independently practicing healthcare provider for telehealth services.

Find Injury Care does not itself provide medical care through telehealth.

The healthcare provider is responsible for determining whether telehealth is clinically appropriate and legally permissible for the particular patient and encounter.

Telehealth availability may depend on the provider’s licensure, the patient’s location, applicable law, insurance requirements, technology availability, and other factors.

Where a third-party telehealth technology provider is used, that service may be subject to the third party’s terms and privacy policies.

Provider profiles may contain information supplied by providers, publicly available information, or information obtained through verification or other sources.

Find Injury Care does not guarantee that all provider information is complete, current, or error-free.

Where reviews or ratings are displayed, users should understand that reviews represent the experiences or opinions of the individuals who submitted them and do not necessarily represent the views of Find Injury Care.

Find Injury Care may remove content that violates these Terms, applicable law, or our content policies.

Find Injury Care may offer providers paid advertising, enhanced profile, sponsored-placement, digital marketing, Google Ads management, or other promotional services.

Sponsored placements will be identified where applicable.

Advertising and platform fees are for the applicable technology, advertising, profile, marketing, or promotional services and are not a payment for a particular medical treatment outcome.

Where Find Injury Care provides Google Ads or other digital marketing services to healthcare providers, the provider remains responsible for ensuring that information about its practice, credentials, services, pricing, insurance participation, and healthcare claims is accurate.

Find Injury Care may rely on information supplied by the provider when creating advertisements or landing pages.

No advertising campaign guarantees a particular number of clicks, leads, appointments, patients, revenue, or other results.

Certain Platform services may require payment.

Prices, billing frequency, applicable taxes, cancellation terms, and other payment terms will be presented before purchase.

Unless otherwise stated, platform, advertising, marketing, profile, and technology fees are separate from fees charged by healthcare providers for medical services.

Find Injury Care does not determine a provider’s professional medical fees.

You agree to:

• Provide accurate information when using the Platform;
• Use the Platform only for lawful purposes;
• Not impersonate another person;
• Not submit false provider or patient information;
• Not interfere with Platform security;
• Not attempt unauthorized access to another user’s account;
• Not upload malicious software or code;
• Not scrape or copy Platform content without authorization;
• Not use the Platform to harass or harm another person; and
• Not use the Platform to obtain or disclose another person’s private health information without authorization.

Your use of the Platform is also governed by our Privacy Policy.

Please review the Privacy Policy to understand how Find Injury Care collects, uses, discloses, protects, and retains information.

Where Find Injury Care processes protected health information on behalf of a healthcare provider as a Business Associate under HIPAA, the applicable Business Associate Agreement and applicable law will govern the permitted handling of such information.

The Platform may contain links to or integrations with third-party services, including healthcare providers, telehealth providers, payment processors, analytics providers, communication providers, advertising platforms, and other service providers.

Find Injury Care does not control third-party services and is not responsible for their independent privacy practices, security, availability, content, or services.

Your use of third-party services may be subject to their own terms and policies.

The Platform and its original content, branding, software, design, text, graphics, and other materials are owned by or licensed to Find Injury Care and are protected by applicable intellectual-property laws.

You may not copy, reproduce, modify, distribute, sell, or commercially exploit Platform content without prior written permission, except as permitted by law.

To the maximum extent permitted by applicable law, the Platform is provided on an “as is” and “as available” basis.

Nothing in these Terms limits rights or protections that cannot lawfully be excluded.

To the maximum extent permitted by applicable law, Find Injury Care will not be responsible for indirect, incidental, special, consequential, exemplary, or punitive damages arising from your use of the Platform or healthcare services provided by third-party providers.

Find Injury Care is not responsible for medical treatment, diagnosis, clinical decisions, medical outcomes, provider conduct, insurance reimbursement, or other professional services provided by healthcare providers.

Nothing in these Terms excludes liability that cannot legally be excluded or limited.

To the extent permitted by applicable law, you agree to indemnify and hold harmless Find Injury Care and its officers, directors, employees, contractors, and affiliates from claims, losses, liabilities, damages, and expenses arising from your unlawful use of the Platform, violation of these Terms, or infringement of another person’s rights.

We may suspend or terminate access to the Platform where we reasonably believe that a user has violated these Terms, applicable law, or Platform security requirements.

Providers may also have separate agreements governing their participation in the Platform.

We may update these Terms from time to time.

When material changes are made, we may provide notice through the Platform or other reasonable means.

The updated Terms will become effective on the date stated in the updated Terms.

These Terms will be governed by applicable law, subject to any mandatory consumer-protection or other laws that cannot legally be waived or displaced by agreement.

Any dispute-resolution, venue, arbitration, or class-action provisions should be added only after review by counsel for the jurisdictions in which Find Injury Care operates.
